WE CARE FOR YOU: Angels On Wings provide quality care to our clients that will allow them to prosper and thrive in the comfort of their home through the delivery of diet and nutrition, personal hygiene and medication management, other ADL’s and assisting in coordination of care between multiple medical providers. Additionally we will ensure our clients safe reliable non emergent transportation services to and from medical facilities, physician's office, dentist's office, pharmacy, suppliers of medical equipment for the purpose of receiving medical treatment or medical evaluation or purchasing prescription drugs or medical equipment.
				Company Objectives: Promoting patient safety by creating fall prevention interventions tailored towards the homecare settings Help clients live with greater independence Improve medication adherence in the homecare population Preventing medication errors and polypharmacy Assisting in improving communication amongst multiple care providers through maintaining updated med list, medical diagnosis, and medical provider list Preventing unplanned hospital readmissions by having knowledge of and preventing risk factors that cause exacerbation of chronic disease Ensure safe and reliable non-emergent medical transportation to and from medical provider appointments using the most appropriate form of transportation
How Can We Make A Difference: The end-of-life process of a patient requires support from the people around them—including family and loved ones. Our goal is to assist clients in achieving physical, emotional, and spiritual comfort, while allowing them as much autonomy and dignity as possible. We commit to our brand of supportive care, where the patient is supported and cared for by an expertly trained and hospice interdisciplinary team.
